Partnering for Seamless Transitions and Better Community Health
At Coastline Family Medicine, we understand that a patient's journey doesn't end when they leave your facility—it is often a critical new beginning. We are dedicated to bridging the gap between facility-based care and the home setting. We actively partner with hospitals, nursing facilities, and home health care agencies to ensure your patients receive continuous, high-quality medical attention exactly where they are most comfortable.
By working together, we can reduce preventable readmissions, ease the burden on families, and elevate the standard of care throughout our community.
Let’s Collaborate
If you share our commitment to patient-centered care and want a reliable partner for your complex discharges, we would love to connect.
How We Support Your Discharging Patients
-
The first 24 to 48 hours post-discharge are the most critical. To ensure patient safety and continuity of care, we offer prioritized, next-day in-home follow-up appointments. We are there to immediately assess the patient's transition, catch early warning signs of decline, and provide immediate interventions that keep them safely at home and out of the emergency room.
-
Patients leaving with multiple new diagnoses, adjusted prescriptions, or mobility challenges require dedicated oversight. Our unhurried, comprehensive in-home visits allow us to:
Perform thorough medication reconciliations to prevent adverse drug interactions.
Assess the home environment for safety and fall risks.
Coordinate directly with Home Health Care (HHC) nurses, physical therapists, and specialists to ensure all aspects of the discharge plan are actively managed.
Provide ongoing chronic disease management and palliative support.
-
Coastline Family Medicine was founded on a simple philosophy: healthcare should be deeply personal, accessible, and community-oriented. We don't just want to be a referral destination; we want to be your collaborative partner in doing good. We are passionate about working hand-in-hand with local healthcare organizations to improve overall health outcomes, advocate for our vulnerable populations, and bring compassionate, proactive care directly to the neighborhoods we serve.
